Position Title: Home Health Registered Nurse

Location: Chestnut Hill Area, PA

Employment Type: Full-Time or Part-Time

About The Role

We are seeking a dedicated Home Health RN to provide skilled clinical care to patients in the Chestnut Hill area. This role requires experience with OASIS-based visits and skilled home health care. Please note: private duty experience does not qualify for this position.

Key Responsibilities

• Deliver skilled nursing care in accordance with physician orders and home health standards
• Conduct OASIS assessments and develop individualized care plans
• Collaborate with interdisciplinary team members to ensure quality outcomes
• Maintain accurate documentation and comply with regulatory guidelines
• Travel within the designated service area to provide in-home care
• Participate in weekend, holiday, and on-call rotations as needed
  
  

Required Qualifications

• Minimum 1 year of clinical home health experience using OASIS
• Active Registered Nurse license in Pennsylvania
• Current CPR certification
• Valid driver’s license and reliable transportation
• Strong communication and organizational skills
• Ability to work independently and as part of a team
